HOOPFEST TEAM REGISTRATION

The following information is needed to register online and "pencil in" your spot in the tournament:

1. Email address of person registering the team

2. Team Name

3. Team Address

4. Division of Play (Grade) and Level of play requested (A/B)

5. Coach/Assistant Coach/Team Contact Info (Name, address, phone number, email address)

6. Any scheduling requests you have

7.  Notification if a coach will be coaching more than one team in the tournament

The team roster should be sent as soon as possible after registering and may be sent via mail or email (or fax with pre-arrangements).   A roster sheet is available during online registration that you can print and fill out offline.  Rosters must be received and payment must be made in full to have your spot fully secured.  If the tournament sells out (it did last year), those teams that we have to repeatedly contact for roster or payment will be replaced with teams who have all information and payment ready.  Thank you for your cooperation.
