2. 2018 MAA Equipment Ackowledgement, Code of Conduct and Photo Consent Form

3. MAA Physical Form

4. Concussion Form

5. Copy of the player's/cheerleader's Birth Certificate

6. Copy of the player's/cheerleaders Report Card  

7. Payment
