LATE REGISTRATION

WEDNESDAY JUNE 19th, 6-7pm

At WHY USA Properties,  4869 S Bradley Rd #104 Santa Maria, CA 93455

Note:  Late registration includes a $20 late sign-up fee

 

Please come prepared with the following at Registration 

• $220 registration fee 
• (2) copies of the child's birth certificate
• (2) copies of their report card
• (2) 4x6 photos of your football player (not needed for cheer)
• Proof of residence
• AND the football player(s) so we can weigh them.  

  IMPORTANT:  Football players that do not sign up by this date (5/31/2013) will not be able to have a name plate on their jerseys.  

NOTE: CHEERLEADERS need to provide the same documentation but do not need to be present to begin their registration process.

ATTENTION returning players/families: Unresolved financial obligations (including unreturned equipment) may impact your registration fees. Those that think they may have these issues should contact the OYFL President or Vice President to determine resolution actions. Thanks!
