SAFETY PLAN FACILITY SURVEY

LEAGUE PRESIDENTS: Many leagues that turn in a safety plan to Little League, do not turn in the facility survey, and their plans are rejected. The facility survey is mailed to your league's address every year along with all of the other ASAP materials. The facility survey is simply a three page form that needs to be filled out about your baseball fields. If you do not have this form, I have uploaded it in a PDF file in the handout section. It is also linked below this paragraph.

LEAGUE PRESIDENTS: I would also like to urge everyone to go to this website http://facilitysurvey.musco.com. This is the official Little League/Musco Lighting Facility Survey Center. All you have to do is login with your League ID# and password. You can fill out your Facility Survey right there and it even gives you the option of printing it out afterwards in a PDF file. If this is your first time using this, you will have to obtain your password from Little League. You can send Heather Anderson an email at heather.anderson@musco.com requesting your password. Just include your name, League ID# and affiliation with the league. I strongly urge everyone to take advantage of this.
