IN CONSIDERATION OF THE PERMISSION GRANTED TO THE BELOW NAMED ATHLETE (S) BY AMBLER OLYMPIC CLUB (AOC) TO PARTICIPATE IN ANY OF IT’S PROGRAMS, I RELEASE AOC AND IT’S AGENTS FROM ALL ACTIONS, DAMAGES, CLAIMS OR DEMANDS WHICH I, MY HEIRS, EXECUTORS, ADMINISTRATORS OR ASSIGNS MAY HAVE AGAINST AOC FOR ALL INJURIES KNOWN OR UNKNOWN WHICH THE BELOW NAMED ATHLETE MAY INCUR BY PARTICIPATING IN AOC PROGRAMS. I ALSO AGREE TO RELEASE AND HOLD HARMLESS AOC FOR ANY SUSTAINED DAMAGES. I ALSO STATE THAT THE BELOW NAMED ATHLETE (S) HAS BEEN APPROVED BY A LICENSED MEDICAL PHYSICIAN TO PARTICIPATE IN ANY OF AOC’S PROGRAMS.

Fees for AOC Spring Development Track are $195.00 for the first athlete and $60.00 for each additional athlete. There are no refunds after March 23, 2020
