Lady Titan LAX Family,

If you do not have a current sports physical on file, LSR7 will be hosting 2 sessions where a sports physical will be performed. A valid sports physical MUST be on file or you will not be allowed on the field on 25 FEB (start of the Spring 2019 season)

Here are the details:

February 2 @ LSWHS (Front Entrance)   
OR
February 23 @ LSHS (Attendance Entrance)
Time:  8:00 am to 10:30 am (doors lock at 10:30) students are seen in the order that they
           arrive.    

Who:  R-7 SCHOOL DISTRICT STUDENTS
            Entering Grades 8th-12th for the 2019-2020 school year

Cost:  Nonrefundable $20.00 Pre-registration online through your parent portal in
PowerSchool (look for ‘make a payment’, add ‘sports physical clinic’ (choose location) to cart, check out)  Online registration will close at noon the Friday before each clinic.  (This is a fundraiser that benefits all three high schools Athletic Training Programs and the cost is nonrefundable.)              
              $25.00-Walk-ins (cash or check only)

*2019-20 HIGH SCHOOL Students only!!  If you already have a PRIVIT account, parents and students please update all documents (archive all prior forms and re-sign all documents for 2019-20 school year) or New to PRIVIT you will create an account on your school’s PRIVIT page.  After creating an account, you will add your student as a member and enter their information. Also, please JOIN A TEAM so that your student’s profile is visible to his/her school activities office. Upload page 2 of the MSHSAA Physical Exam Form after completion of the physical.   HIGH SCHOOL STUDENTS ONLY: Go to your high school’s PRIVIT page

to access PRIVIT, complete your information, and upload the form.  CAUTION: If you enter your information on the wrong high school website PRIVIT cannot transfer the information.  
**Please print and bring a copy of your Pre-participation history form from your student’s PRIVIT profile with you.

*2019-20 MIDDLE SCHOOL STUDENTS!! —DO NOT LOAD YOUR INFO TO PRIVIT! Please have pages 1, 3, and 4 of the physical form filled out before your arrival.  You will need to turn your physical into your Activities office at your middle school.
These physicals will be good for the entire 2019-2020 school year and for the rest of this year.
