Centennial Girl's Lacrosse

Reminder that your paperwork is due this FRIDAY Jan. 29th. Meet outside the locker room after school 2:15-2:40 p.m.

If you who played a winter or fall sport, you only need a blue permission form.

If you did NOT play a winter or fall sport I need the following:
1. proof of residency (BGE Bill, Comcast Bill OR Copy of your Deed)
2. permission form (this is the blue permission that must be signed by you AND your parent)
3. a medical form (this is the pink form that must be filled out AND signed by a doctor)
4. Copy of your birth certificate (Freshman ONLY).

The blue Permission form and the pink Medical form is available right outside of the girls locker room.
