I’ve begun receiving materials back from some of you — thanks. Although my original deadline was 7/31, it would be ideal and a huge help if you could return the forms this week instead of waiting till the 31st so that we can submit to the registrar before she leaves on vacation.
If you have NOT received an envelope in the mail from me containing registration form, medical release and player passes, please let me know immediately.
(thx to the following families: Gurney, Logan, Wolfe, Colevas, Bengoechea, Ramos and Krasko. I’ve sent you individual notes if info is missing. thx, Wagner — please advise if you’ve since received forms in the mail).
Also — a few very important reminders for filling out the forms and player passes:
1) EACH PLAYER and a parent must sign the registration form.
2) A parent must have the medical release form notarized (don’t forget to fill in the date of last tetanus booster — if you sent forms back but forgot to include the date, please email it to me and i will add it)
3) Each player must sign her player pass IN BLACK INK in two places. (legal first name, last name only – as it appears on the front of the card). If you signed in blue ink, I have to re-print and the player has to re-sign. Please remind your girls BLACK INK.
