Hi everyone,

Yesterday I mailed out paperwork to you. Please review it and return it to me at your earliest convenience, but before July 31st.

- Each player and a parent must sign the registration form.

- A parent must have the medical release form notarized.

- Each player must sign her player pass in black ink in two places. (first name, last name only – as it appears on the front of the card).
