We are now accepting mail-in registration for all leagues for our 2012 season. To enroll you child, please complete the registration form above and submit it, along with the following documents to Warren Youth Baseball:

A copy of your registration form with your appropriate league fees
A copy of child’s birth certificate
A completed Waiver/Release form
A completed Code of Conduct acknowledgment

The Waiver/Release form and Code of Conduct acknowledgement can both be downloaded found in the documents section of our website. When completed, please mail all documents to:
