Rookie League (Ages 5-6) $50 League Fee
Minor League AA (Ages 6-8) $75 League Fee
6 year olds with one year rookie experience may apply for AA
Minor League AAA (Ages 9-10) $75 League Fee
Major League (Ages 11-12) $75 League Fee
Junior League (Ages 13-15) $75 League Fee

REGISTRATION DUE by February 1st, 2012, after this date Late Registrations will be $100

Registration DUE by April 15th, any application received after that date will be put on waiting list.

FUNDRAISER SALE or BUYOUT $100.00
Candy Sale will be approximately 144 Seroogy’s bars per player at $1.50 each. In lieu of selling a set of boxes per player, a buyout option is available for $100.00 per player, to a maximum of $300.00 per family. The buyout payment must accompany the registration fee and the application.

VOLUNTEER DEPOSIT IS $100.00
Each Little League family is required to pay a $100.00 Volunteer Deposit. There are No exceptions. The Volunteer Deposit must accompany the registration fee and the application. Parents or Guardians must volunteer FOUR ( 4 ) hours per player in the Little League Concession Stand;
to a maximum of 12 hours per family. Parents or Guardians must also volunteer FOUR ( 4 ) hours per family to Umpiring or Groundskeeping.
Two umpired games will satisfy the Umpiring or Groundskeeping volunteer time. ALL volunteer hours must be supervised to count towards the volunteer commitment. A Volunteer Form must be completed after volunteer hours have been performed. AFTER ALL VOLUNTEER HOURS ARE FULFILLED, AND AFTER THE REQUIRED VOLUNTEER FORM HAS BEEN COMPLETED AND MAILED, YOU WILL RECEIVE YOUR DEPOSIT BACK.

RESIDENCE REQUIREMENTS
South Milwaukee, Cudahy and Saint Francis and Bay View residents are eligible for Regular Season and Post Season play. Residents of other communities without a nationally chartered Little League are eligible for regular season play only.
Residents out of boundary who have the continuity clause – (ie started with SMLL and then moved) eligible for post season play.

Waiver:

I, the undersigned parent or guardian of the above applicant, hereby give my approval to my child’s participation in any and all Little League activities during the coming season. I assume all risks and hazards incidental to such participation, including transportation to and from the activities; and I hereby waive, release, absolve, indemnify and agree to hold harmless the local Little League, Little League Inc., the organizer, sponsors and supervisors, for any claims arising out of any injury to my child.
I agree to return the uniform and any equipment issued to my child in a condition comparable to that as when received. I understand that I will be charged the cost of replacing the uniform if not returned, or if replacement is necessary due to the improper laundering, willful abuse or similar neglect.

I recognize my child’s responsibility in the Little League fund raising activities for this 2011 season. I am aware that each player is expected to sell his or her allotment of candy and that refusal to participate in the candy sale or “buy out option” will result in my child being removed from his or her team.

Consistent with a manager’s ability to conduct the affairs of his or her team, a manager may disqualify a player, subject to a Board of Directors approval, for the current season from the team if the player repeatedly misses practices or games. I understand that SMLL cannot be held responsible for any incomplete or inaccurate application.

I recognize my child’s photograph may be used by South Milwaukee Little League publications, brochures and web sites.
