2016/17 Online Registration Form

Steps to Register online:
1. You must have your childs USA Hockey Number in order to complete this registration. Please go onto USAHOCKEY.COM to register.
2. Fill out the online information form for EACH child as instructed and submit. Please note we are no longer accepting credit cards as payment.
2. Print registration form and send payment. .
3. Fill out Jersey and sock form if needed.

Waiver & Release Form

I do hereby give my approval for the Participant listed above to participate in the Morristown Jr. ColoniaIs ice Hockey
Program. I fully understand that there are many inherent risks, direct and indirect in this program. Being fully aware
of these risks, I am willing to permit the Participant, whom I am legally responsible for, to participate in the program. In addition, I am willing
to assume all risks inherent in and incidental to such participation, and I hereby release, absolve, indemnify and hold harmless Morristown Jr. Colonials Ice Hockey Program-organizers, participants, and coaches, of any claim arising out of any injury, to me or the Participant. I hereby authorize and
request Morristown Jr. Colonials Ice Hockey Program to act on my behalf according to the best judgment of Morristown Jr. Colonials Ice Hockey Program under prevailing circumstances in the event of any injury, or in the event that I am not
able to act for myself, or the Participant, or I cannot be contacted.

Image Release: I hereby grant to Morristown Jr. Colonials Ice Hockey Program the right to use all
photographs and videos taken of the Participant during any Morristown Jr. Colonials Ice Hockey program for website, advertising and promotional purposes.
